The questionnaire will be considered at a parent forum related to the
School In Need of Improvement Planning process to be held at the
Cooperative Middle School Library Tuesday, November 16 from 4:00 to 5:00
PM. If you are a parent of a student in grades six through twelve at
either CMS or EHS and would like to provide feedback, you are invited to
attend. If you cannot attend this session and would like to share your
thoughts with the committee, you are encouraged to print out this form
with your responses and mail it to:
